1. confirm that the information you provided is true, correct and complete to the best of your knowledge, contains no willful falsifications or misrepresentations, and that you are authorized by the organization you indicated in the Application to submit this Application on its behalf;
2. consent to Encana Oil & Gas (USA) Inc. ("Encana") collecting and using any of your personal information that is contained in this Application and any other personal information that has been or which may be subsequently disclosed by you to Encana or any of its representatives to consider and respond to this Application and for Encana to communicate with you on subjects and events relevant to Encana's Community Investment Programs; and
3. agree that you may be contacted to provide additional information, to set up a meeting, host a site visit or present to an Encana review committee.
